What is Home Health Care

Heart of the Rockies Home Health is here for you. We are a Medicare-certified home health agency providing healthcare services in your home after a debilitating illness, surgery or hospitalization. Our home health team works directly with your physician to plan your care and we keep your physician informed about your progress.

Our Home Health team consists of case managers, registered nurses, licensed practical nurses, physical, occupational, and speech therapists, medical social workers, and home health aides. Each has a role in your plan of care.

Case Manager

At Heart of the Rockies Home Health, each home health patient is assigned to one of our experienced Case Managers who is either a registered nurse, physical therapist, or speech language pathologist depending upon the skilled services required. Their role is to develop the plan of care and rehabilitation goals with the patient and medical team.

An overall health assessment is conducted and the case manager designs a plan of care with you, your physician and other members of the home health care team. Continual oversight by the case manager helps you meet your health goals and to prevent hospitalization. In addition, education is provided to you and your caregivers for self-management of care once discharged from home health services. Frontier’s philosophy of care is based on a holistic approach that considers your wishes and health goals.

Nursing

Our nurses will make the first home visit and assess your needs. The nurse works with you, other members of the team and your physician to develop a plan to meet your needs. Nursing services include:

  • Assessment and management of medical conditions
  • Technical procedures such as colostomy and catheter care
  • Medication management
  • Infusion therapy
  • Wound care
  • Draw of blood and other specimens for lab analysis
  • Patient, family and caregiver education

Physical Therapy

Our physical therapists (PT) work with you after hospitalization, a stroke, orthopedic or other surgery or for any medical condition that affects your strength and your ability to move around safely. Physical therapy services include*:

  • Fall prevention training
  • Strengthening exercises
  • Transfer and gait training
  • Weight bearing and balance exercises

Occupational Therapy

Our occupational therapists (OT) help you regain independence in self-care activities. Occupational therapy services assist you to: safely bathe, dress and use the toilet; improve strength and flexibility; assess the need for safety devices. Occupational therapy helps you:

  • Regain ability to dress, bathe and toilet
  • Gain strength and flexibility
  • Safely use adaptive equipment

Speech Therapy

Our speech language pathologists (ST) help you with speech, communication or swallowing problems related to your condition. Speech therapy services include*:

  • Swallowing exercises
  • Techniques to prevent choking
  • Speech retraining
  • Memory exercises
  • Communication techniques

Medical Social Workers

Medical Social Workers (MSW) provide information about community resources specific to your condition or situation. Social work services include*:

  • Referral to community resources
  • Information about facility placement, and financial resources
  • Assist with advance care planning, including living wills, advance directives and POLST

Home Health Aide

Our home health aides provide personal care services, if needed, during your recovery and while you are on home health services. Home Health Aide services include help with*:

  • Bathing
  • Dressing
  • Light meal preparation
  • Toileting
